What happens next?
- Submit your request online or call the front desk.
- Your request is not confirmed until Meridian’s front desk contacts you.
- The front desk confirms the department, provider, location, date, and time.
- If appropriate, the front desk will let you know whether an in-person or telehealth visit is available.
- Meridian generally serves patients age 16 and older.
Not every Meridian service can be booked online. If you do not see the service you need, please call.
