MEDICARE PATIENTS
Annual physical or Medicare Annual Wellness Visit?
A Medicare Annual Wellness Visit is different from a routine annual physical. If you are covered by Medicare, contact Meridian before scheduling so we can help identify the appropriate preventive visit.
Benefits vary by insurance plan. Tests, treatments, or problem-focused services completed during the same visit may have separate copays, coinsurance, or deductibles. Check with your plan about preventive-visit coverage.
Prepare for your appointment
- Bring a photo ID and insurance card.
- Bring a current medication and supplement list.
- Bring any forms that must be completed.
- Bring relevant records and vaccination information.
- Write down questions you want to discuss.
