Preparing for Your Annual Physical

A little preparation helps your Meridian Primary Care team review changes in your health and plan appropriate preventive care.

Preparation checklist

  • Bring your photo ID, insurance card, and any forms you need completed.
  • Bring an updated list of medications, vitamins, and supplements, including doses.
  • Note changes to your medical, surgical, and family history.
  • Bring available immunization and outside screening records.
  • Write down health concerns or questions you want to discuss.
  • Follow fasting or medication instructions only if Meridian gives them to you before the visit.

What to expect

Your visit may include a review of your health history, medications, preventive screening needs, immunizations, vital signs, and an examination appropriate to you. Testing, vaccines, forms, or additional problem-focused care may depend on the appointment and your coverage.

Check your coverage

Check your plan for preventive-visit coverage. If you are unsure whether you scheduled an annual physical, a Medicare Annual Wellness Visit, or another visit type, ask Meridian before your appointment. Services discussed or performed during the visit may involve cost-sharing depending on your plan.
